Who is a candidate for outpatient therapy?
You qualify for in-home outpatient therapy services if:

You live in a private home, senior housing community, or assisted living facility

You have a need for physical therapy, occupational therapy, or speech therapy

You have a physician's referral for therapy services

You are not considered "homebound" as defined by Medicare
-
Traveling to our outpatient clinic would be difficult, risky or inconsistent

The focus of your treatment is on functional activities in your home environment: ADLs and IADLs

In-home caregiver training is needed due to dementia or other special health conditions or concerns

You have recently been discharged from a hospital, rehab center, skilled nursing facility, or skilled home health service and would benefit from continued outpatient therapy at home

Your goal is to age in place and you have a strong desire to remain in your home for as long as possible
How do I receive High Thrive Therapy services?
High Thrive Therapy accepts new clients by referral only.
You may be referred by a physician, nurse, social worker, community agency, home health agency, personal care aide, family member or you may self-refer.

Once we receive your referral form, we determine if our outpatient therapy services are adequate and appropriate for your needs.
We will contact your primary care physician (PCP) to obtain
a physician's order and approval for your treatment plan.
Prior to your initial appointment and evaluation, complete the New Client Intake form.
Our therapy services are reimbursed under Medicare Part B, Mainecare and PPO (Out of Network.) We also accept private pay.
We take great care to process your intake quickly so we can schedule your first appointment as soon as possible.
