Mobile Outpatient Therapy

No worries! If it's inconvenient, challenging or risky for you to visit our clinic, we will simply bring the therapy clinic to you at home! 


Mobile therapy is outpatient therapy that occurs in your home rather than at an outpatient therapy clinic. Mobile therapy is for anyone that may have a limitation on ability to attend in-clinic treatment sessions, such as transportation issues or a preference for in home care or per physician recommendation.

You do not need to be classified as "homebound" to qualify for at-home outpatient rehabilitation under Medicare Part B. There are many benefits to using our mobile outpatient therapy service and we are pleased to meet you wherever you call home: your house or apartment, senior living community condo or with family or friends.

Getting Started

The path to wellness starts with a referral for outpatient rehabilitation services with High Thrive Therapy!

A caregiver, aide, family member or friend can refer you and you can also refer yourself.

What To Expect

Once High Thrive Therapy receives your referral for rehabilitation services, we will contact your doctor or healthcare provider to confirm your diagnosis, prognosis to ensure that outpatient rehabilitation will benefit you and their provider referral orders.

We will confirm that you are not currently receiving home health care services.

 

Next, our scheduling coordinator will contact you to discuss available dates and times for therapy.

 

Your therapist will arrive at your home at your scheduled time with a mobile therapy kit that includes all the clinical tools, gear and equipment to conduct your therapy sessions at home.

 

Your initial evaluation to determine your goals and create your individualized plan of care takes approximately 75 minutes to complete. Therapy sessions thereafter will take 55 minutes.

 

 All evaluations and treatments will be performed in the privacy and comfort of your home. Treatments may also include education and training for loved ones/caregivers if needed.
